Please make sure before printing any of the following forms, the margins in your browser's Page Setup are not more than .5 inch for right and left margins and .5 inch for top and bottom margins. Please be aware that different computer systems have different font managers for both the screen and the printer. The correct screen resolution for the printable forms is 800x600 pixels using small fonts. With the correct resolution and margins you may still not get a well formatted printed form.
